European Practice in Gynaecology and Obstetrics is a series of books published under the auspices of EBCOG. In each volume, the specialist will find a review of basic science, recent concepts in pathophysiology, clinical aspects, imaging, treatment and unresolved problems or controversies, and up-to-date references . Endometrial carcinoma is the most common genital malignancy in the Western world and is the second after cervical carcinoma in developing countries. It is considered a disease of women over 40 years of age, with 2/3 occurring in the post-menopausal age. Most endometrial carcinomas are diagnosed in early stages and are associated with favourable outcome. The prognosis of advanced diseases is poor. Compelling data suggest that there are at least two pathogenic entities of endometrial carcinoma, one that is oestrogen related, and the other that arises from atrophic endometrium. Management of endometrial carcinoma has changed during the last two decades, mainly as a result of the introduction of surgical staging by FIGO. Controversies in performing proper surgical staging, however, exist. There are a number of other controversial areas that are discussed in this book. Similarly, recent advances in diagnosing, managing and screening endometrial carcinoma have been highlighted. One of the major steps made is the inclusion of lymph node dissection in the surgical treatment of uterine cancer. This is not a consensus book, but rather a collection of the views of experts. Guidelines are provided for practising gynaecologists based on current evidence, along with the opinions of the contributors to this volume. We do hope this book will be of value to all involved in the care of women with carcinoma of the endometrium.
